This is about people, not pets

India carries a large share of the world's human rabies deaths, and rabies is effectively always fatal once symptoms appear. It is also completely preventable if treatment starts promptly. The gap between those two facts is mostly people not realising that a small scratch counts.

Washing is not a formality

Fifteen minutes under running water with soap removes a large share of virus at the wound before it can reach a nerve. It is the single most effective thing anyone does after an exposure, it costs nothing, and it works best in the first minutes.

Do not apply turmeric, chilli powder, oil, lime or any household remedy. Several of these are traditional in India and all of them make the wound harder to clean properly. Do not close or tightly bandage the wound either.

The categories, and why the third one is different

Category I is touching or feeding an animal, or a lick on skin that is not broken. Category II is nibbling on uncovered skin or a minor scratch with no bleeding. Category III is any bite through the skin, a scratch that drew blood, saliva on broken skin or on the eyes, nose or mouth, and any contact with a bat.

The difference that matters is that Category III normally gets rabies immunoglobulin infiltrated around the wound in addition to the vaccine course. That is the part specific to the most serious exposures, and it is the part people miss if they treat a bite as minor.

A vaccinated pet is lower risk, not no risk

If the animal is your own and its vaccination is current, the risk is genuinely lower. It is not zero, because vaccination status is sometimes wrong, records are sometimes optimistic, and the consequence of being wrong is fatal.

The decision belongs to a doctor who can see the wound. Telling them the animal was vaccinated is useful information, not a reason to skip the visit.

What people get wrong

  • Deciding a scratch is too small to matter. Broken skin is easy to miss and it changes the category.
  • Applying turmeric or chilli to the wound.
  • Waiting to see whether the animal falls ill. Post exposure treatment is time sensitive and observation is not a substitute.
  • Assuming an indoor pet cannot transmit it.
  • Stopping the vaccine course partway once the wound looks fine. The course is the treatment.

Questions people ask

What should I do immediately after a dog bite in India?

Wash the wound under running water with soap for fifteen minutes, starting straight away, then see a doctor the same day. Do not apply turmeric, chilli or oil, and do not close the wound.

Do I need rabies injections for a small scratch?

If the skin is broken at all, yes, that is Category III and it normally needs both vaccine and immunoglobulin. A minor scratch with no bleeding is Category II and normally needs the vaccine course. Only contact with unbroken skin is Category I. Small scratches are frequently underestimated.

My dog is vaccinated and it bit me. Do I still need treatment?

Speak to a doctor. A current vaccination lowers the risk substantially but does not remove it, and the consequence of being wrong is fatal. Tell the doctor the vaccination status and let them decide.

How soon after a bite must rabies treatment start?

As soon as possible. There is no fixed deadline after which it is pointless, and treatment is still given days later, but every hour of delay matters. Do not wait for symptoms in the animal or in yourself.

Is rabies treatment free in India?

Anti rabies vaccination is available at government hospitals and many municipal health centres, often free or at nominal cost. Availability of immunoglobulin varies more, which is a reason to go to a larger government hospital for a Category III exposure.

Can I get rabies from a cat or a monkey?

Yes. Any mammal can transmit rabies. Monkey bites are common in Indian cities and are treated the same way. Bat contact is treated as Category III even without an obvious wound.

Reducing the contact in the first place

Most bites happen on the street, and most street encounters happen during walks. Vaccination is the first line and it should stay that way. Cutting the number of unnecessary trips out is the second, and it costs nothing to do.
